For those who fear "exploit", this is actually a detriment, because +1 potions do not count towards the quest completion, so they worth way less than the normal ones.
By using the +1 supplement you gave yourself a very small chance that your result would also be +1. The same will happen if you were to use +1 materials. And this happens regardless of your focus value as you have experienced.

So, I don't have the screenshots from it, but I tried making potions before with like 6% and it got +1 4 times out of 6. And I have never encountered a 0% focus item going +1 just because +1 reagents.
Sadly, my experience has been contrary to anything following 'rules' as portrayed in the Professions Tutorial for Mod 15+. I've found the chance of getting a +1 result has nothing to do with +1 materials or otherwise (although they help) but is purely a 'one of the next 80 times this user pushes the button he will get a +1 result ... or he will fail completely [with a plus one result showing in the failure message box]).

Granted, it was not for potions per se, but the mechanics determining the results and the lack of logical control of them, have caused me to try only make things that are 100%/100% as long as I can given the maximum tool/supplement range. With higher MW levels, I will have to deal with the failure rate as it is and just go with the flow.

And yet you did use a +1 tool. In my experience, using anything +1 in the process has a chance to yield a +1 result regardless of the focus value.

Aside from some of the early builds on preview, I have never received a +1 result when not using any +1 tools, materials, and/or supplements and being below the minimum required focus of the recipe. I have also never received a normal item when my focus value is above the maximum focus of the recipe.
