Loadouts getting stuck. Multiple characters.

I'm having problems with my loadouts on multiple characters. Mainly the ones that can dual spec. Currently my cleric will not change loadouts between the 2 I have and is stuck on my arbiter build. I have nothing on cool down and make sure I'm in a fire/rest area before I try to change. But it won't work.

I have also had the issue with my warlock. I got stuck on my soulweaver build loadout and could not change back to my hellbringer loadout. In the end I had to retrain my soulweaver loadout to hellbringer. So I have only 1 loadout now on my Warlock as I still can't change it back to the other dps loadout.

I have a Palladin and a fighter who are dual spec'd but now just stick to tank as I don't want the same thing happening to them.

Any known issues making this occur?

I'm having the same problem. My barb is stuck on tank. I applied EVERYTHING to all load outs and still can't change. Video below.

Slightly different but related to loadouts, When leveling up a character and you switch to a loadout with new powers, those new powers cannot be selected until you login and out of the game or change zone.

Oh wait, my loadout became stuck just today. To fix it I did an "apply to all loadouts" on everything. equipment, mounts, and companions. After applying to the mounts, I could swap again.

UPDATE: after unequiping all three items in my quick slots I can change loadouts again. I removed all three at the same time, so I'm not sure if a single one did it. If the game logs this type of stuff it happened between 19:50 and 19:55 EDT 9/13. After reequiping the items I can still change loadouts. I wonder if it's Lliira's Bell, I have two, one in quick, one in inventory.
