Allosaur Companion "Insta-Dies" in AI Basement and in Skyhold

This happens every time I have this companion summoned and enter either of these two areas. I have tried it on two different toons, and it happens on both, every time I enter these zones.
1. Summon Allosaur Companion and enter
a. The basement of Acquisitions Incorporated ( either descending the stairs, or by coming up out of the sub-basement quest areas.)
b. Pirates Skyhold - swampy area where the 3rd/4th MC nodes are located.
2. Immediately get "Your Companion has died. Please summon"
I resummon once I get into the sub-basement quest zone, and it is fine... until I complete and return... and Insta-die.
I have a capture on my XBOX account dated 4/30/2019 GT=panthersnbraves
