[PC] Warlock Hellish Rebuke Not Generating Soul Sparks

While playing my warlock hellbringer with No Pity No Mercy Class Feature I noticed it sometimes (usually boss fights and heroic encounters) does not generate Soul Sparks at all. Since it is kind random, I could not replicate it easily, but it did happen twice at the dragons in The Well of Dragons. Is anyone else having this problem?
