Undermountain Quest "A Revenant's Arrangement"

A few bugs experienced while playing the Undermountain Quest "A Revenant's Arrangement" with my Barbarian character.
While transformed into either Xanathar and Undertaker my weapon was not showing during combat or idle. (using a Bloodspiller transmuted into "Savage Maul of the Many Arrows")
Additionally it seems my character appears as a Xanathar while fighting the Xanathar and appears as a Undertaker while fighting the Undertakers, which is the opposite of what the quest states.
