Hard Core Dungeoneer Achievement not advancing

Achievement steps are not completing as Dungeon is completed, in HC mode. So far only Cragmire Crypts has indicated the completion, all others have not. Tried multiple ways, with and without the inclusion of item level restrictions, tried different days, after the weekly reset, after the patches and still not progressing. Am i doing something wrong?
