This bug has been in game since I think the mods release. Usually it doesn't bother me as I just have to die, the npcs reappear, its super quick. But today I died, reset the quest instance twice, reset the map instance twice before it worked.
The bug happens after you clear the first area and are asked to clear the second. You go down the hallway with the swinging axes and into the room with npcs. The issue is you clear out the npcs, but one is either hidden or doesn't appear thus never setting off the trigger for the other group of npcs to drop down from the back corner.
You're only options are as above, kill your self and run back from the beginning, leave the quest instance and reenter, or as I had to do today, change instances entirely.

There is no sure fire way to fix this but a few other options may work. Icy Terrain can hit the mob. The 'ghost hit' from Weapon Master's Strike can damage it. There may be other powers. I have sometimes had success by staying close to the wall but just outside of what would be melee range. Sometimes if a mob can't pathfind to the target that they are aggro'd on, they may teleport to it, coming out of the wall. Some of these methods can work on that one drake in the Cult Prison as well.
Logging out and back in has always worked for me
My big gripe with HoC is: Does the Elder Nightspeaker really need 58 trillion HP?!! It doesn't make him hard, it just makes him BORING.
