I don't remember the color. The one to the far left if you're standing with your back to the temple/entrance. Is this going to happen again?

The black head bugs if people cross the line before it rises, or according to some, attack it in this time. Either way, not crossing the line should prevent anyone from attacking and so prevents either condition.

Tell everyone to wait for the cleric to yell "she is weakened!!!!" and you're good.

And if you are lucky and it doesn't bug when someone does this, the chest will sometimes bug instead where no one can claim the chest at the end.

From what I remember, that one occurs if you finish off the last head within the last 5 seconds of time.

You still win your weekly cache/Linu's/random loot, but the chest won't open due to the game thinking you didn't win that round (ie, didn't finish off all the heads in time).
