Item lvl issues

I have been having issues with my characters losing item scores randomly without changing any items on my characters. This has only been happening since this last addition of the Cloak Assendency for Xbox One. Has anyone else been having these issues,

Is you companion summoned? It is the most common cause of item level lose.
