This generally happens when you requeue within the instance. Did you rerun without people leaving the skirmish?

yes we rerun it without leaving.becouse we have no idea that we need to exit or leaving to get the banner. we didn't know it was a bug

I'd be pissed if that happens to me but I knew already it is bugged for some time now. They should have looked into that a long time ago. Maybe you'll get it post patch automatically without redoing it...doubt it though.
