I think what this game lacks is accessible fun dungeons. Exept T1 wich are run a bit more now that there easy for everybody, when it comes to T2s only spider and CN are run "frequently". I do ECC and Grey Wolf from time to time with alliance members and such but everytime i do them i understand why peoples don't run them much, they take twice as long and don't give better loot. And even tough for our alliance there not "harder" as we can breeze troug them pretty easily, they do are harder for mid tier characters.
In T1s there all easy but Malabog is so long nobody want to run it, and Valindra ain't run that much either. It seem its either Kessel or Shores for quick easy AD or its Lostmauth wich is timewise very decent and rather easy.
Then there is the rest.... FBI is so annoyingly hard that its barely run. Its hard for DPS to get into runs, as usually those spots are fill fast and peoples want to run this dungeon with 2 clerics and 1 tank. From what i heard Spellplague is even harder.
I understand they is content destinied to endgame characters, but even some engame characters can't run it... and other engame characters just don't want to do it cause the reward vs the chore of doing it is not worth it.
I myself completed FBI a few times... between the annoyance of switching gears to enter it, the chest glitching at the end and not letting me open it, i just decided to not run it anymore...
I have yet to unlock spellplague.
I think this game need another 2 or 3 Spider dungeons, something fast, not too hard, giving good salvagable gear to spice things up.

I would love to see new dungeons and skirmishes. Ideally one of each for each standard campaign. I mean, why two Underdark skirmishes, and no Dragon campaign skirmish? Why no Icewind skirmish?
Graduating levels of difficulty matched with equal and appropriate graduating rewards.
The issue would be numbers of players and queue time, but that might be addressed by revising chest keys.
Have the standard (though graduating) AD reward with loot, and a locked chest with an additional reward, but that can only be opened with a Zen Dragon key, or key constructed from the campaign progression table. Most campaigns have this, but many don't. Also have a reward chest for standard dungeon keys that can also be used with Zen Dragon keys.
People won't want to use two Zen bought keys on one dungeon, but WILL want to open both chests. But since they can only make one key per day per campaign, they might diversify more, rather than shower, rinse and repeat the same dungeon six times in a row.
For Skirmishes, use the Underdark model, with standard reward and campaign key/dragon key chest.
I think the expectation is, that when you are 2k you run with higher characters.
If they make the entry reqs reflect the minimum you need to be for all players to run it succesfully;
a) there'd be far fewer people running dungeons,
and
b) the dungeons would quickly become too easy for those eligible to enter.
I've been in groups not much better than 2k who played as a group and completed EToS, and equally been in with mainly 3-3.2k ego maniacs and spent three quarters of an hour watching the same three speed merchants dash into encounters solo and die.
I think that, as a player, it needs to be understood that if you enter anything at the minimum required level, you are going to find it tough going.
