How does Bitter Cold work?

I found this thread from 2013, though not sure if that's how the feat works at the moment (given the current description vs. what the thread explains).

So, a few questions:

1. How does BC work with powers like Conduit of Ice and Icy Terrain? Does it keep refreshing as long as the target is affected by them (i.e., if BC ends, but the target is still affected by CoI or IT, does it get reapplied?)
   
2. Does it increase damage from all sources (party members, queue group members, companions, etc.)?

targets affected by a chill get 5% more damage. debuff for everyone. ALso ray of frost can activate it and chill strike anything with chill.

So, does Bitter Cold get auto-refreshed? As in, as soon as BC falls off, if the target is still affected by Chill, does it get reapplied? Or does BC require a fresh stack of Chill (as in, previous chill stacks have to fall off, and a new one applied)?

Also, is there a target limit? Esp. with Icy Veins or Chilling Control (with Sudden Storm having no target cap).

I'm about to respec, and so, don't have a way of testing easily/cheaply =(.

1 chill is enough for the mob to get the debuff yes it refreshes but do not stack.
