Both my characters cannot change instance in Icewind pass, get the red message wrong type of map or in combat. This is stopping me from getting groups.

It's not a bug. You cannot change instances in Icewind Pass or Dwarven Valley since they are also PvP zones and the number of players of each faction, 10 tens or arcane brotherhood, is attempted to be balanced/limited.
Simply return to the city and join a group from there, when you travel to the pass or valley you will be in the instance of the group. Generally, you must also be the same faction to group with someone.

I didn't notice it say anywhere in game to form your group in Caer first. None of the other zones are like that so it would be useful if when you try to change instances it explained why you can't and suggest forming a group first.

Even though this is obvious to experienced players (and a reasonable restriction to keep oPVP from becoming a stompfest), it's definitely not presented intuitively in-game and it's not explained clearly anywhere that I can find. I too was confused, and I was fortunate to have a player explain it to me, as I have done for countless others since.
As Zeb mentioned, the only real way to "change instance" in an oPVP zone is to party up with somebody who is in the right instance and then try re-entering the map, hoping it drops you in with them.
@fenumren I bet that it's harder to change than you think, and if they fiddle with it they might inadvertently break instance changing in other maps... Personally, I'm for leaving it as-is and maybe adding some better explanations in-game.
