Dungeon Party Bug

Whenever I queue for a dungeon (doesn't seem to matter which one), I get placed in an instance where I'm the only one in my party; there is a second party with the other players in (at least I'm guessing they're in a party together). This means that, even if I complete the dungeon, I get no credit for it, because the other players get the credit and as far as the game is concerned, I wasn't in their party.
1) Log in with L70 (2.7k) Oathbound Paladin
2) Queue for any dungeon (mostly tested here with Cloak Tower and Cragmire Crypts)
3) Accept the dungeon and enter (when everyone else accepts)
Observed Result
There were no other players in my party, but two other players in the dungeon with me
Expected Result
To be in a party of three players running the dungeon together.
I've been playing since April 2014, and it's worked fine in the past. This has been happening since yesterday, and appears to be permanent now (have tested more than a dozen times). I have three characters, and this is only affecting one of them.
Spec (just in case):
Quad Core Intel i5-6400 2.7Ghz
16Gb RAM
Radeon R9 380X with 4096Mb RAM
