Paladin: Stuck In Combat Mode

Issue: Paladin becomes stuck in combat stance after combat has ended and no enemies are within aggro vicinity. During this time I am unable to perform any action requiring use of the A button on the controller such as looting chests or exiting maps. This does not happen every time combat ends but does happen so frequently I feel this is an issue that needs to be addressed. This issue has been confirmed by other players and seems to affect the Guardian Fighter class as well. Any help or insight into the issue is greatly appreciated. Thank you.
