Not in a party when entering a dungeon

When I queue for a dungeon and a match is found... i enter the dungeon (ToS for example) and im with two other guys.. but we are not in the same party... im alone, when we kill the last boss , Im supposed to get some AD or blue items (not from the chest, from the boss). I dont get anything. and the chest gives me just the blue item ( i should get more items through the dungeon, but no)

It only happens with one of my characters, and only in dungeons, skirmishes work well

I have seen this happen several times we call you a greenie because your name and info show up green instead of blue and you are not listed as a member of the party, it's really strange.

It happens all over the place. I personally had it in Dongeons (e.g. VT), skirmishes (e.g. Kessel) and PVP (e.g. Gauntlegrym). It seems like the queue system does not move you to the right instance and you end up in PE. The rest of the team get's some random guy as replacement.
