Not sure if this is the right place to present ideas but I'll do it anyway.
Some day ago I had a discussion with few friends and we came to an interesting conclusion. PVE aspect of the game can be made a lot more fun and more competitive with rather simple changes.
1. Before entering any epic dungeon - allow team to adjust hardness of the dungeon (basic for t2 is 2000 item lvl - allow them to increase it as they see fit; making opponents have more hp, def and atk), which may slightly affect the reward they get after they are done.
2. Make a leaderboard with parties(party members names) that managed to clear highest item level in each dungeon.
Short and simple.

However rewards wouldn't be changed much but would only grant certain amount of AD per dungeon - but, that amount would vary on the hardness set by your team (erm, 15000AD for completed 3000item lvl dungeon or more and 40000AD for completed 4000 item lvl dungeon or more) and would only be granted on first dungeon in the day(so you would have to choose wisely not to set dungeon too hard risking the fail and not getting any AD on that day).
P.S. I'm not really sure with the ad amount; its just the wild guess

You have some that run content with pre-mades an BiS and complain it's too easy.
And others getting 1-hit killed and wiping at the first sub-boss.
And usually the people with advantages tell the ones struggling they suck or don't know how to play their class, yata yata.
Variable, optional, difficulty & reward scaling sounds really nice, but the work to execute that would be pretty big.
Considering old missing dungeons that require a blanket retooling of difficulty are still missing. Doubt this will ever see the light of day.
