i wont add balance changes/bug fixes since its permanent process
1)RP - leave just unbound and acc bound version of normal RP stones(peridot, aquamarine etc.), remove thaum, power/stability etc
2)PvP - new modes and maps, considering the small pvp player base focus on some 5v5, 10v10 ctf, resource nodes, you could even make foundry contest with some zen rewards for new maps
3)Add personal(account wide, could be too expensive to build a house for every character) housing - 1 or few map markers that allow to travel to a base location you can build on, building could be either foundry like free build or preset housing options(for ppl who dont want to build every detail, to add more houses types you could make foundry contest with top votes getting their houses added to item shop and getting zen rewards. building could take resources, some existing could be used(ores, wood, cloth etc.) that would add value to those resources and allow new players to earn some ad while leveling. huge profit options selling stuff for houses and house designs, like furniture, armor stands etc. this instance should be permanent so players arent kicked out like in strongholds
3)Professions - complete rework, make professions account wide and add workshops to player houses with actual moving npcs(alchemists in labs, blacksmiths in forges, leadership patrolling your house), based on the persons and assets you have. ppl would be inclined to buy profession assets and persons simply to make their workshops look nice. Leadership could provide unbound vouchers, and, maybe some campaign items to reduce campaign grind. And professions should be more resource based than just time gated with about every resource there is being completely worthless(except for dragon eggs and such)
4)Foundry needs much more options, chance to make dungeons(with a choice from premade bosses based on foundry type - solo, 3man, 5man, and, maybe, 10man, with some changes to queues. each type should have normal(or t1 for group content) epic and legendary difficulties), it should be like custom games in warcraft3 or dota, with some reasonable rewards - RP, marks, gear based on difficulty. and contests for best foundries with zen rewards to creators
5) Dungeons - we need more dungeons and a legendary difficulty - for the top 5% for challenge and some a bit better rewards(like +2 ilvls and a bit more stats, just like black ice upgrades or reinforcement on t2 gear) maybe some weekly dungeon with good rewards that takes 1 hour or just giving weekly status to a certain dungeon that gives extra rewards on 1st completion
6) AD sinks - they need a workover since we will have much less ad in he game(mount/pet upgrades, additional bonus unlocks for artifact weapon/offhand, profession sinks etc.)
7) More packs, in-game titles, forum titles, cosmetics from Zen store - earn money on stuff ppl want, not on forcing ppl to buy shortcuts because the original content is too boring, or on overpowered stuff like legendary mounts
8) Improved communication and monthly/per-module state of the game videos where devs explain what and why they are doing and where they want the game to go, smth like this -
http://www.youtube.com/watch?v=gZ29AjhyoYQ (i hope this video is ok since this is completely different genre)
9) make the game more alt friendly, atm its too hard to play 2 or more characters, at the start, before artifacts it was much easier - run some dungeons and get gear, buy some rank5~7 enchants, lesser weapon/armor enchant and you are good to go, now you need a ton of rp, grind all the campaigns for a month+ its just too much work
10) dungeon based progression instead of RP based one - ppl want to have and run dungeons not grind some boring stuff like dragon hes and rp - run a dungeon, get an item and be done with it, instead of spending a lot of time adding rp to an item
Might have forgotten something, but these are my main issues with the game, that considering its really well done action combat system and rich lore(at least for the D&D franchise) could be rally successful

I think something has to be done, in terms of costing, grind time ect to allow for players to playalts again. I think its alot tied into RP , TOO MUCH rp is needed to progress a toon. Decrease the amount of RP or make it drop more in game and viola we are in the game again.
Then, certain people said "Hack! This is boring". Then, they increased the difficulty, infinite grinding, etc. All these made alt unfriendly.
Yes, I have to admit that in certain stage you could beat everything without being anywhere near to BiS (such as 50% BiS). Yes, I have to admit that was "kind of" boring. However, when I hit the "boring" point, that was my time to make a new alt and to have fun again. That was the time having 10+ alt was considered "casual" legitimate players.
