Someone made a character with all the Doctor Destroyer parts; it's kinda like the person running around as a Mega-Destroid. Heck, I've seen people run around as Defender.

When a hero is standing just feet away from his Nemesis
I don't think Dr. Destroyer considers Defender a nemesis or threat of any kind. Shadow Destroyer was only a concern to him because he ousted him with dumb luck.
