The damage you take from Knight's Valor is slightly delayed, so you likely got resurrected by your soulforge proc, then took KV damage from someone who had the buff before you died, which killed you again.
For old content this is largely true but it does not apply to nearly the same extent for newer stuff. Most anything (that does not re-use an old area) added from mod3/4 and onwards have traps that are entirely invisible.

Over the last ~4 hours I've had a total of five RP item drops in Drowned Shore, while questing and killing mobs pretty much non-stop. If you fail three times. One shard given per crate opened, and one shard per weapon salvaged. All classes have five weapons, three of which were included originally and two more which were added with mod 5, for the peregon path specific at-wills.

Everything will be getting better, unspecified bugs and issues will be fixed, the devs are aware of them. Nobody touched the hoard enchants in any way, hoard coffers will drop more and the quest turn-ins require less. Heralds will be buffed once Tiamat arrives.
It might be fine on low-health mobs where it can't be stacked, but on high-health targets like HE dragons it's absolutely insane. Far worse than SW Tyrranical Threat or CW Oppressive Force non-stop HV stacking was. Until it's fixed, any non-TR non-SoD user is pointless in HEs if there's a TR (ab)using SoD there.
