I can't be 100% sure of this bug, but... here goes. I've used 15 preservation wards to upgrade the quality of my artifact from green to rare, which has a 50% chance to happen. The probability of failing 15 attempts is of 0.003%, fair to say it's impossible to do what I did if it was really based on luck so I'm assuming it's a bug.
Now, I noticed that I had some reagents in my overflow bag; when putting these out I was finally able to refine my artifact normally on my first try. So I'm guessing you can't consume the overflow bag reagents even though the option to refine shows up and even though the preservation ward is consumed. Pretty annoying, but yeah!
It may not be a new bug, it's really not unusual for a 40 or 50% chance upgrade to fail 10+ times in a row- whether that is malice or a really poor distribution by accident I can't say.
While it sounds like mathematically-challenged magical thinking, I remember someone posting that whenever they get a failure fusing, that they always stop or try to fuse something else, because of the insane frequency of failure streaks, contrary to the stated probability. I know that it sounds like boneheaded folk wisdom, but it really works for me (and I'm pretty sure it isn't just confirmation bias). There are only so many times that you want to burn your whole stack of green wards on a 50% chance, especially since unless you pay zen for them they are that much harder to get.
I'm aware of the trick to wait after 2~3 failures, it actually does work and I've been doing that when trying to upgrade my artifact. Another thing that I noticed was that I had to click the Upgrade button twice after putting my preservation ward, the first time it wasn't doing anything (but I did get the warning window). At first I assumed it was just lag, but... yeah, pretty sure it's a bug.

It felt off from the beginning. I too noticed the strange behaviour with the confirmation box.
First you click accept to go ahead and attempt with the upgrade and the box dissapears, but nothing happens.
After that, you must press the upgrade button again and it fails every time.
After reading this thread, i checked my overflow bag and found one of the reagens in it.
I moved the reagent to a normal bag and the upgrade succeded the second time after that.
I am totally convinced this is a bug
Just glad I used preservation wards instead of coalescent ward

Items in your overflow bag should not be valid to interact with, and normally need to be moved to regular inventory space in order to do anything with them (sell, consume, use for refinement points, etc.).
So yeah, I'd say the bug is that the option to attempt the upgrade shows up under these circumstances, until the components are relocated to normal inventory.
That's true, but...I wonder if the overflow bag isn't quite working properly after the winter festival. I was able to turn in fish straight from my overflow bag during the festival without taking them out of overflow -- and yes, I was given lights for each stack out of the overflow bag. Lately I've been selling stuff at vendors without removing it from overflow because I figured out that I could drag it from overflow straight to the box on the vendor window, and it would sell successfully without having to take it out of overflow -- and yes, I was paid the proper amount of gold for it. I thought it was working as intended, but after reading this I wonder if the bag isn't working right.
Prior to mod 2 I was able to use preservation wards from the overflow bag while fusing enchants, without any uptick in failure rates, but again I don't know if being able to use them from overflow is technically intended or not. I didn't notice where the ward was in my inventory until afterward so I just kinda rolled with it.
Wasted 14 Wards by 50% upgrade chance. Then I realized that one of my ingredients was in the overflow bag. I put it in the regular bag and my refinement was successfull. Definitely a bug.
