I had the same problem, but sussed it out. It appears that my graphics card control panel thing wasn't recieving some of the program default settings from the game. so, from my nvidia control panel i set all the global settings to awesome and hey presto... it works.

Happened to me too today. Joined queue, entered the game and the whole thing was failed, the whole thing and it was still pulling in other players when I was leaving. I left, and got a leavers penalty for an hour. No fair.
