Defense rating is bugged and displayed negative numbers when sitting still, defense is also bugged when speeding up and much lower numbers since last patch.
Superior Romulan Operatives have still not been fixed and not giving full bonuses
I often wonder how effective the accuracy vs defense vs accuracy system is, myself, in addition to what you're saying. I do know that if you aren't at Red Alert, those numbers aren't the correct value of your bridge officer and ship passives and Captain traits aren't engaged. That being said, the shield registration output always seems to increase when shield power is set to max even when not at Red Alert.
I've also left my ship power level to max engines for extra defense but seem to still get my T6 BOP (from Klingon recruitment) pounded.
Some people are far too sensitive online while others are far too crude. Because Internet anonymity.
I will say I have noticed some similar issues with speed related boff abilities not stacking appropriately... As a PVP player, I will look into this more in depth... as for your post lost user, in regards to NPCs, do not credit them accuracy... if they hit they hit... unless you are clear over 100 defense which 97% of players are not, you will get hit. It also greatly depends on your ship class as well. In a dreadnaught, you are getting hit unless you have intel team on and/or other significant boosts to defense attribute...
This report is accurate and impacts xbox as well... haven't not looked through PC, but I tried this morning to get ahold of someone over there and have not heard back... i'll try to let one of the biggies know... as for the defense issue, here is what i posted in the xbox support page...

I WILL SAY, there is an issue with base defense being WAY off...between -2.5 and .75 average on an escort.. with 2 points in defensive maneuvering... and sitting still obviously, is just incorrect... and throws off further scaling during max combat speeds... that said. tested all the boff abilities and they are working fine, as are the specs and the majority of the gear I ran across my ship... I will say there is a number in the 2015 math 24* that idk... without a fair base to start from... my defense does seem incredibly low compared to where i remember it... hitting 80 and peaking like 90 just seems way off in my adamant that is pvp built... im used to seeing like 150-200... so.. besides the base stat failure there may be scaling math errors as well, but i presume this is unlikely...
