Ultimate Tech Upgrades did not improve quality/ Mk and missing

Good evening gentleperson(s),
Earlier yesterday evening I had attempted to use an Ultimate Tech Upgrade (UTU) to improve the quality and Mk level of the Delta Alliance Bio-neural Gel Pack. While the resulting product did show the result to be Mk XV-Epic, upon retrieval, it reverted back to Mk XII-Very Rare. Additionally, there were two (2) UTU's remaining, one (1) used for the 'Gel Pack and should've had one (1) UTU remaining. Both have since been unavailable and are now also unlocatable.
A quick checking of my account will provide evidence of recent purchases of five (5) Key Ring Bundles with only three (3) UTU's being used for the Delta Alliance Hyper Efficient Engines, Delta Alliance Warp Core and the Delta Alliance Deflector Dish, respectively. This allowed for there to be two (2) UTU's to be held over. One of the those "two" was used to upgrade the 'Gel Pack' when this issue became present.
Please address this and advise ..

This appears to be more of a Glitch and may not affect others. I was able to successfully upgrade the 'Gel Pack' via the Starship Status menu. The losses occurred when upgrading via the Inventory menu.
