Klingon's voice when I accept hail from Adminal Quinn (Xbox One)

When I hit Commander Rank, I got a hail from Admiral Quinn, but the voice was that of a Klingon and he referred to Q'Nos

    Same thing happened to me today. Also, I got the hail and the ship, but my rank is still showing as Lieutenant Commander. I only noticed when I went to the tailor to update my rank pips and wasn't able to.
