The following is copied from a Ticket I wrote to Arc Support and re-posted here as per their request:
I am unable to switch my second or third costume slots to Off-Duty, Club, or anything other than Uniform, even after the game asks me which type of outfit I would like to equip in that slot and I try to change it. I am unsure if this is intentional, but it does not seem like it.
All three slots are stuck on Uniform, and when I choose the others, it only allows me to browse through Uniform options. To my knowledge, the first slot is reserved for Uniform only, but I didn't think that was true for all slots.

Second response to Arc Support's follow-up questions:
I have tried changing at multiple tailors. All three slots seem to be locked on Uniform.
The drop-down menu for selecting different outfit types is not grayed out.
To my knowledge, many other people are complaining about this issue in the online chat, so it does not appear that my character is the only one that is affected.

Thank you for the additional slots recommendation, skullblits. I have heard people say that buying additional slots is the workaround, however, if those slots ever get used for a Uniform item, they too will become locked on Uniform. Even the workaround is still prone to having this issue crop up, which is why a better alternative (an actual fix, for instance) should be made available. I am of the opinion that spending money to fix a simple programming error should not be the acceptable solution, but I understand if others are more agreeable to that.
In a similar vein, for a long time people told me to purchase a Captain Retrain Token in order to combat the fact that I could not spend any skill or specialization points that I had acquired, stating that the developers would not fix the issue. In time, they did fix it. Granted, it was over a month later, but they did get around to it, which I am thankful for.
I am by no means an apologist for Arc or Cryptic, as they have also done a lot to garner the ire of their console community, myself included. However, I'll wait and see what the next patch brings. If the update for Agents of Yesterday and their Anniversary Event provides a resolution for a fair number of issues, I'll be somewhat sated.
I do have to say, the Support Ticket system seems rather useless. I've submitted many of them, and never once have they done anything more than admit that what I'm experiencing is a "bug", and that I should post on these forums. At this point, I merely use the Ticket system as a formality; a chance for them to show me they can actually accomplish something without just referring me to the community forums. Thus far, they have failed me.
