Summary:
Based on my observation the Invincible trait bug works like this:
If a ship uses the invincible trait its hull needs to be reduced to a value *between* 5% and 0%. else the buff as well as the cool down does not trigger and the target stays invulnerable. (a strike which brings the hull from over 5% to equal 0% doesn't cause the buff and cool down to trigger but it prevents the target from exploding.)
Details:
If a ship using invincible (the zahl starship trait) has >5% hull and gets hit hard so the hull drops immediately to 0% invincible doesn't trigger but the ship is invincible. Invincible doesn't trigger until the ship heals up a bit to >0% and <5%. That enables one to stay at 0% for a long time.
I've prepared a short video which shows this behavior. I've managed to keep a shuttle alive for around 110 seconds at 0% hull.

I've seen it happen on my ship for around 30 seconds in pvp too, with all around me shooting at me and preventing any hull regeneration for invincible to activate (I suspect the hull has to stay under 1% for that to happen). If I'm able to break free and regenerate my hull from 0% to over 5% in one step, invincible doesn't activate at all and there is no cool down so I can use that trick again without waiting.
This is shown in the following video. (To see the videos in better quality watch them on YouTube might help reading the buffs)

I don't have the code insight but it looks to me like there is something like a >0% hull instead of a >=0%hull mistake somewhere. Probably the reason the invincible buff doesn't trigger at 0% hull.
I have to add that I don't use the temp spec tree with continuity… and I did the above test several times with different spec settings so I suspect those have no influence on this.
Even if I can use this as a big advantage, I beg you to fix this because this trick is just too strong to keep the fun in pvp. The "Zahl Trait" is already strong enough when it works correctly. (maybe it should be removable by buff removal like subnuke, to get it a bit more in line with other stuff.)
I hope this helps fixing this bug.
Big thanks to Bastet and Teiwaz for helping me with the test videos.

But I don't know how many actually knew the way to make it proc.
Maybe if there woud have been a better communication between devs and pvpers ... oh wait wrong season.
After all of the money people threw on that "I win" trait ... are you f. kidding me? LOL
Lol I will second that
