Per canon there are two Rahm-Izads, too, if I recall correctly. And too many species whose names sound like "Tarellian."

Not to mention the half a zillion species of Rigelians that were thought of before ENT brought us canonical Rigelians.

There are also two planets called Krios, both introduced on TNG. One was a Klingon subject world seen in "The Mind's Eye", the other was a planet that had just ended a centuries-long war with Valt Minor, as seen in "The Perfect Mate".
It's been suggested that these two planets Krios are in fact one and the same, but I fail to see how Krios could have been waging its own war against Valt Minor while Krios itself is already ruled by the Klingons (and indeed, was fighting against the Klingons for independence in "The Mind's Eye").
Edit: The planet Krios from "The Mind's Eye" was then retroactively renamed "Krios Prime" when revisited in Enterprise episode "Precious Cargo". Though Memory Alpha has separate pages for the two planets, the Krios Prime page seems to conflate the two planets' histories somewhat.
Is it really that hard to believe in coincidences for the series that gave us the Romulans?

Remember they said in Voyage they take great pains to keep their Theta Radiation away from their homeworld, so being in the Beta Quandrant and dumping in the Delta Quandrant sounds about right to me.

A) Warp drive isn't fast enough for that. It would take decades for them to get there and back.
The sector space tooltip for the Malon system would have mentioned native inhabitants. This is the sum total of what it says:
Comprise of four planets in close orbit around a Class-B star, the Malon system is dominated by Malon Prime. The Planet is a L-class world noted fpr the crystalline structures in the western mountain ranges. Artists carve the larger crystals into abstract works of art that are prized by collectors throughout Cardassian space and the Federation.
The Malon system in Beta Ursae has nothing to do with Space Hooker Chemical.

In a very large galaxy, one would assume that theres a chance more than one planet has the same name?

The Malon star is probably a Federation Standard transliteration of a Cardassian or Bajoran word that refers to something completely different.
