Today I upgraded a Romulan Plasma High Density Beam Rifle Mk XII [CritX]x2 to Mk XIII. The upgrade to Mk XIII completed properly, but the weapon also lost its modifiers in the process.

Without the modifiers, the weapon is much less effective after being 'upgraded'. I don't know if this is specific to this exact rifle, or if all romulan weapons are vulnerable to this bug. I don't intend to find out because you are basically throwing away upgrade items until it's fixed.
I ran into the same issues when I upgraded a mkXII Romulan Plasma compression pistol to a mkXIII, upgrading to ultra-rare quality;
The pistol lost its modifiers, but gained a CritH modifier, resulting in an ultra rare mkXIII Romulan Plasma Compression Pistol [CritH] That doesn't quite seem like the proper number of modifiers.
My experience suggests that this is common to all Romulan Plasma weapons, but that it doesn't affect fleet weapons with [CritX] (thankfully). Also worth being careful with anything with the [borg] mod, as slotting the weapon into the upgrade system at all strips it off, even if you don't do anything else with it.
As a follow-up: I've upgraded romulan plasma weapons on three characters, with two of them upgrading [CritX]x2 weapons and one upgrading one with no [CritX] mods. The one with no [CritX] mods upgraded like you'd expect, the ones with [CritX] mods lost them. When you go to upgrade these new weapons the projected result is an ultraviolet weapon with a single [???] mod. If you slotted the weapon that upgraded normally the projected ultraviolet weapon is [???]x3, which seems wrong in that I would expect it to display the previous mods plus a single [???] mod.
The characters affected by the [CritX] loss are Tirozed@myrnwyn and Aiheth@myrnwyn
One more thing related to this, although I forgot to check if this applied to the weapon that didn't lose mods, is that the little quickbar icon that shows what weapon you have selected claims that these Mk XIII weapons are Mk X when they're equipped.
Note to be careful with Romulan Plasma Ground Weapons as even putting them into the upgrade system will strip the Crit[X] mods off them. Lost them off a MK XII Splitbeam Rifle on my char V'ael@veiltih.
I imagine since this is a live beta we won't be getting any refund, as I doubt Cryptic will be able to fix this post fact? I've upgraded my Romulan Plasma Pulsewave Assault Mk XII [CrtiX]x2 and got a Mk XIII without any modifiers. That's not only dilithium wasted for the upgrade, but the item itself...

Kinetic Cutting Beam Mk XII loses Dmgx3 mod when clicking upgrade item as well... Even more dilithium wasted...

Add Vulcan Lirpa from Hearts and Minds to the list - even selecting its upgrade completely removes its unique modifiers, turning it into a generic rare mk xi lirpa.

Tribble testers can't fix the bugs we can only report them and hope they are corrected by the design team. I'm pretty sure this issue was reported in the Tribble official upgrade feedback thread, frankly it should not have been pushed to the live server in the state it was in on tribble. The current tribble build still has upgrade issues of which these were 2 of many upgrade problems reported.
Just adding that I also lost my [CritX]x2 modifiers when I upgraded my Romulan Plasma Pulsewave Assault MK XII. Too bad I didn't read this posting before I tried it.... rats. Submitted in game bug report as well.
I went to upgrade my elite fleet plasma warp core slotted it in but changed my mind, when i used it next i had lost 40-45 dps on my weapons, bloody things useless now.

Edit: Okay then, decided to try it, and it has been fixed, successfully kept the CritX x2 and added a third modifier upon upgrade to UR.
