I've seen a few reports of this, but I've also noticed what seems to be something new.
When I beam down to a planet, sometimes (rarely) my away team appears underground and the mission cannot be completed. Sometimes, even the MOBs drop through. This seems to occur more often when there is lag or latency in your internet connection.
I recently had a laggy connection due to ISP problems and every time I beamed down to a surface, my away team fell through the ground. This has also happened when I had a good connection, but nowhere near as much, or as repeatable.
1: Beam down and the entire team, me included falls through the ground. BOs die from it. Mission can't be completed.
2: One or more BO just vanishes into the surroundings, rocks, underground and it may or may not be required to cancel the mission. Most times it is as when this happens, 9 our of 10 times, enemy NPC's do the same and you can't defeat them or complete the mission.
I sent reports on these events several times, dating back a few months now. Never got any kind of reply.
Oddly, I also wasn't experiencing any lag at the times it occurred.

I've never seen the first problem, but the second has been all too common for me. I've filed three or four bug reports on this starting sometime in October. Of course, that was before I realized that there doesn't seem to be any way to recall the ticket number of a bug report after the fact so I don't have the numbers handy. I would have thought that particular feature would have been a no-brainer for any bug reporting system. Silly me.
I tend to have the same problem, except in the foundry. I've been working on a Tholian mission, and whenever I attempt to leave the spawn point, my away team and the Tholians fall out of the map. I can't finish testing the mission until this bug is solved.

From what I understand the pathing grid for NPCs is only created upon publishing a Foundry mission: that might be the issue.
i've gotten #2 a couple of times. the last time it happened, i ended up defeating all the mobs without the aid of my away team and felt like a boss. ^_^
not sure if that was the intention of the devs though.
