I'm a bit confused about exactly what took place during the Ash Tyler/Voq "species reassignment surgery". I also realize that the description given in the show might also be vague and/or inconsistent. Anyway, here's my take on it:

Voq's body was surgically modified to resemble Ash Tyler (possibly using tissue or organs from the "original" Tyler). Then an engram of Tyler's consciousness and memories was imprinted on Voq's neural pathways, over Voq's own personality.

In essence, what we're left with is Voq's body with a copy of Tyler's brain. Does this sound about right?

It also seems possible, depending on what was done to the original Ash Tyler, that he might still be alive somewhere?

    I think your assessment of the situation is correct; that is Voq's body, but with Tyler's mind, or personality and memories at least. I don't however suspect the original Tyler to be alive; if your assessment is correct, and his organs were used for Voq's transformation, it seems unlikely the Klingon's would have bothered with keeping the original Tyler alive; he probably ended up served at a Klingon buffet.
