the game is alive, though of course if many of your old friends are no longer around you might need to look for new ones or just indulge in single player (includes PUG queues)
lots of content for you to check out, if you are bored after that take a break again. STO is not going anywhere. in a month or two the summer event starts, so you might come back then to grab your free T6 ship.
other than that, same old with a few new features on top.
A lot has changed in two and a half years, including being able to create TOS era characters, new space sectors opened up, yet more reputations. and improvements (perhaps) to lighting and graphics. Some players have left but others have joined!
And if you want to become rich beyond your wildest dreams of avarice--you're probably a Ferengi.
You'll have a couple of new reps to grind, and new Fleet holdings that need completing. The game is experimenting with new sorts of multi-player queues, and lots of people hate the new queue UI. (I'm not one of them.) There was a revamp to certain abilities lately, and mostly the commentary on that has quieted down (or I'm deliberately avoiding those threads).
It's good. I spend two hours a day just mucking about every day--which is all the time I have, given my daily schedule. It's certainly viable for that at a minimum.

Run like heck away from this game. It is so much worse than 2014. Luckily you did avoid Delta Rising...the BEST EVER and the smaller useless seasons. Overall it is in declined just being fed by platform players who sooner or later will drop off as much PC are too...

Always growing and expanding. Lots of new stuff since 2014. If you like STO as a Star Trek CRPG its totally worth to come back.
Depends a bit on why you went away. If it was bug fixing reaction time.. well it still "if it does not crash the game its working as intended" Cryptic we are talking about; so think thrice before opening your wallet

My reason for going away is...well, each season was putting better stuff which was making existing stuff obsolete (yes, the stuff you grinded for), and simply the daily tasks were becoming kinda repeating...not to mention playing lottery with boxes and doffs (no, never paid real cash just zen -> lobi conversion). The lobi prices were also going up due to market being flooded with zen etc.
Glad to know that more content is available. I'm just going through the forums and feel the temperature
