Veteran Reassignment Form? Where do I get it?

I have been wondering about this for the last year. I go to the bridge trainer and looked at the special officers and see these veteran bridge officers (which tells me I have 0 reassignment forms).

How do I get these Reassignment forms? This was barely mentioned in the internet and the only thing I got out of it is if you spend a form on a officer and if you decided to remove him... then you are not going to get him back. But funny thing is, I haven't even got one form yet for any of my characters.

    The Veteran BOff was awarded to players during the conversion to Dilithium during Season 5, so you won't get one without a time machine.

    If you find a time machine, by the way, can I hitch a ride? I want one of those BOffs too...

    The veteran boff is one of the rarest items in the game and nearly impossible to obtain, if at all anymore. I heard they could be traded early on but I doubt there's one anywhere now that isn't bound. Even if you could find someone who has one, I can't imagine what the asking price would be. I would be curious to know though.
