The only information I could find on receiving the "Borg Liberator" title is that you need to destroy all 3 dreadnaughts within the allotted time without someone on the team dying. I would like to confirm whether or not this is true and if there is a requirement to play it on advanced or elite to obtain the title. Thanks in advance.

I'm very happy to be corrected, but I thought it was for ensuring that none of the Cooperative ships were reassimilated (possibly, reaching the towers to begin reassimilation) in a mission - as opposed to killing the dreadnaoughts. It could be both, however.
For those interested, I have confirmed that the "Detour to Freedom" accolade (Prevent any Borg ships from arriving at the assimilation platforms) does not give you the "Borg Liberator" title. Unfortunately, we missed the "Assimilation Cancelled" accolade (Prevent any Borg from being re-assimilated) by only 2 ships, so I cannot confirm that one.
You need to kill all three-dreadnoughts, have no re-assimilated Borg/rescue all the Liberated Borg AND no teammate can die during the course of the mission.

Satisfying all those criteria will be a bit tough for me since I mostly pug, but, hopefully, I'll be able to get it eventually.
All the more reason to keep playing.
Update: Somehow, I received the title without meeting all the aforementioned optionals in a single session, so I'm somewhat at a loss. I'm just guessing, but you may be able to obtain the title by gaining the required accolades in multiple sessions. Either that, or perhaps the mission stat info displayed at the end is bugged.
