How do you get a 4th hot bar?

I have seen some people on twitch or in screen shots with a fourth hotbar. How do you get it to show?

Press escape, you'll then have an option to reorganize the UI. On the bottom of your screen, you'll have an option to add a 4th bar near the BOFFs skills location.
