Yeah, I would agree that you don't want to be upgrading just the basic ground stuff. Make sure you're using your traits and buffs wisely. Keep track of what missions give you gear rewards you like (or just go back and hit replay on them to see what they rewarded). When you reach the next tier for equipment, go back and rerun them as they should be level appropriate. Until you hit the specialty XII+ stuff at 50+ though, all that stuff is pretty much throw away. Not to mention that there's some superior ground stuff you can get from fleet stores that's probably cheaper in terms of dil.
so the item rewards, do they level up with you? if i was lvl 50 and did one of the early missions, would i still get the same items as last time or would i get upgraded ones?

For the most part yes, you would get an upgraded one. But it might be slightly different.
On these mission, you can see you get the same base personal shield, but the mods are different depending on what level you are at the time you do the mission. That's because of the typical NPCs you're up against at those levels.

ok what about missions where you are using an alien ship, can you strip it of its items and keep them before you warp out of system or what have you, or will they just disappear?

You get the same item but a higher mark version.
Unfortunately, you can't keep that stuff.
