Yeah, there's nothing that can prevent such a thing absolutely. There will always be the random risk of something going wrong. Such is the way with everything in life.
Still not nearly as bad as the Vanguard satellites in the 1950s where only three succeeded out of eleven launches, or the Ranger lunar impact probes, which had six failed missions before the first success. This is, after all, rocket science.
