So I finally got around to checking the assignments on guys, and instead of the 20+ that should have been there - each was sporting 15-17+ instead. The "+" would be any Sick Bay stuff. I've got no idea what they would have been, I might run certain types but I don't really pay attention to the specifics of those types...and when there are no more of those types, I just fill them out with whatever is there to fill the 20 assignment slots. So uh, yeah, I can't be much help there with what was actually missing for those 3-5 assignments each guy lost.
Just wondering if anybody else noticed something like this or heard anything about it.
What bothers me isn't missing DOFF assignments, it's the holes that appear in my inventory.
I keep the bottom packed solid, with no gaps, and all empty space at the top. Then after a patch, there are often 3 or more gaps in my inventory, at the bottom. What's missing? I have no idea, but something obviously is.
I just don't trust STO anymore. if "Once bitten twice shy", then what shall we say after being bitten a dozen times or more?

I think what's happening now is the assignments are completing en-mass now, instead of 3 at a time, at least that's what it seems like. But, the missing assignments thing, look at your success/failure rate. Unless I did REALLY well on ALL my toons last night the failure assignments are being auto-acknowledged now.
If this is the case it's not bad in an of itself until it comes to tracking chains (yes, you can always check the chain tab to see if it advanced) and personal tracking of doffs (if you don't see the failure screen you don't see if a doff is in red, i.e. dead, and if you don't see that the only way to find out which one died is to go through the whole list!)
The dead doff thing is the part that really bugs me. I keep track and have a general idea of how many of each department is needed so that I have an idea what ones are free to donate/sell/give to the fleet. If I have to go through my entire roster every time I want to do one of those things I'm probably gonna stop doing it.

I would like confirmation on whether this is intended or a bug. Borticus previously claimed that having so many assignments complete simultaneously put too high a load on the server, so assignments were changed to complete one at a time. But people complained that this was too tedious, so Cryptic changed assignments to complete three at a time as a compromise.

Has anyone had a mission failure where a common DO gets killed and the system does not give notification? That might be interesting to figure out.
The lack of notification would be disturbing, especially since the assignment log is no longer sorted in reverse chronological order. There's no way to distinguish old failures from recent ones.

I have started missions with a full roster of doffs, and found one missing afterwards.
Also, sick bay missions show up when I have not ordered any, so they must be from injuries during failed missions.
I was about to log off then I noticed there was an assignment due to complete in a few seconds... tick, tick, tick, tick, POOF! It vanished into the aether as soon as it completed.

I've noticed it too, less completed missions awaiting me than I had set up. I only log onto those alts 1/week, so it was very glaring that they were all at 17 completed instead of 20. I figured the last patch did something.
I didn't see the patch notes where they stated that they went from the smaller chunks (3-5 at a time) back to where it is now - the way it was for the longest time.

This really needs to be fixed quickly...
Sick bay missions are automatic when a doff is injured during the mission and survives. The higher the mission risk, the higher the chance of getting injured. (they made uncommon and higher doffs unable to actually die during missions a while ago. Common doffs can still die)
There's also a few missions where if you critically fail the mission, your doffs there will be assigned to the brig for a short time.
This isn't a bug, but the game doesn't really explain the system in great detail.

I expressed my doubt that the servers really couldn't handle the task. I thought their code was unoptimized.
