In which mission/level do you get awrded the quad phaser cannons?

    You only get the quads by buying the Sao Palo defiant off the c-store

    Not from a mission, but from the Defiant Refit from the C-store. 1500 zen
    This program, though reasonably normal at times, seems to have a strong affinity to classes belonging to the Cat 2.0 program. Questerius 2.7 will break down on occasion, resulting in garbage and nonsense messages whenever it occurs. Usually a hard reboot or pulling the plug solves the problem when that happens.

    the romulans and klingons also have zen ships with quads. the quads on the qib look good when they fire.

    True, but the OP asked about Quad Phaser Cannons.
    The Klingon and Romulan variants are disruptor and plasma.
