I was playing around with the "Alien Race" which is an option for all factions of the game. It allows you to have a very high degree of customization; including the traits. You can make an "alien" look like a human but have different traits. There should be an option to add "Trill spots" as well. You just need to play around with the various option.
I probably spent about 30 or 45 minutes just playing around with customizing an alien out of curiosity. I didn't bother keeping what I created though.
Okay, so let me get this straight, if I've got a human character, if I want trill spots I'd have to create a new "alien" character and level it up etc?

Yes. Or make a Trill/Joined Trill character.

A Joined Trill costs 500 Zen I believe. Looks just like any other Trill, but the abilities / traits are different. I think Joined Trills makes for a good science toon, but I could be wrong on that since it something I never bothered looking into.

Well yeah because Humans don't have Trill spots, by and large due to them not being Trill.
It really is that simple.
You're just a machine. And machines can be broken.
Nitpicking is a time-honored tradition of science fiction. Asking your readers not to worry about the "little things" is like asking a dog not to sniff at people's crotches. If there's something that appears to violate natural laws, then you can expect someone's going to point it out. That's just the way things are.
Romulans can have Trill spots. Under "complexion" you can select "host", I believe. I made my Rom toon half Trill (in appearance) using this method.
This is not 100 percent correct. If you are making a Romulan (as in the species, not faction) "Host 1", etc is not available to be used.
That said, the alien gen is the only way to make a non-Trill, with Trill spots. The option you want is Host 1, Host 2, or Host 3 (I believe all are available in the alien-gen).
Not sure why you want a Human with Trill spots though, since visually that is basically a normal Trill, which you can easily get by just being a Trill. Normal Trills are free for anyone, only the Joined Trill are a store Purchase (and like was already said, visually the Joined Trill are no Different then the normal Trill)
Whaaaaaaat? It cannot be! Next you'll tell me I have to be Klingon to have forehead ridges and an uncontrollable violent streak!
Well, technically you can be a human and have an uncontrollable violent streak...

Looks like he is half-Zeltron, half-Cardassian, half-Trill, half-pirate and half-musketeer. That's 3 halves more than physically possible.

Not with the Federation's amazing healthcare system!
It's streamlined online!
That's the greatest idea ever! We should totally fund something like this in the real world! What could possibly go wrong?

If you want trill spots, make a trill character, period.
We do have a tail. It's vestigial, but it's called the coccyx, also known as the tailbone.
There are those who walk among us that do have exposed/external tails...although at this point of evolution it is seen as a 'birth defect'.
Hey, I'm still dreaming for catgirls :P
