Phased Tetryon is a dual-proc weapon, so the secondary (phaser) proc takes the place of one of the mods. The best you're going to get is [CrtH]x2.

(Voth AP and Nanite/Elachi Disruptors fully replace the normal procs of the weapon type, hence why they have the normal 3 mods)
