Current known Missing and Buged Uniforms and Outfits

this is what I have gather so far.
The Klingon Adapted honor guard helmet is missing from the selection screen.
The save outfit feature does not save the outfit and will load random features to the selected Costume slot. (I currently know this problem for my Klingon character. if any one else knows this problem for other factions please post below.)
The Omega XII costume set is currently missing for my Romulan character. I have unlocked it threw the reputation system and still it remains missing.
Using the any of the special costumes such has Honor guard or Omega. when purchasing it then exiting out of the tailor menu and go back to alter it, it resets the whole outfit to a random uniform rather than loading the previous purchased features.
that is all I currently know, please post additional information below.
