it usually not balanced, but thats why
its called pvp.

Balance isn't a question of player skill, it's a question of the various combinations of available gear and abilities having adequate counters or if not, reasonable effects to reflect the fact that there is no defense against them.
I don't really understand your 3rd point.
I've seen and heard stuff about 'battleground' style pvp stuff, I guess similar to Ker'rat and CnH, but unfortunately, I just can't see it coming any time soon, looking at the state of the game

I can safely say PVP is not one of those things that "everyone wants to do..."
Maybe a few more people would participate if it was improved in some way, but not everyone equates "difficult challenge" with "enjoyable gameplay experience"... there are many ways to enjoy playing the game and there's no right or wrong way to enjoy what you do here, assuming what you do doesn't interfere with another individuals enjoyment.

they should create 1 area a sector block for all pvp needs.
and with that they can add all kinds of restrictions
for that sector block, ranging from ships to levels.
it would be alot easier then having a private boot camp.

Making a new sector map for pvp would be very time and resource consuming. Based on the current amount of people who pvp it doesn't seem likely that Cryptic would make that money back very quickly. They are more likely to make things which they know the majority of the player base will want/use.

I hope that Cryptic some day understand that in order to buy the new ships that fill they wallet, we need to have a place to use them. And a decent PVP (not grate, not fantastic, just DECENT) is the only end game content that it never ends.
But I guess that until ppl keep buying the new shiny ship to make the same STF over and over again, that is not going to change.. ever.

sector block, 8 planets, 12 maps, 2 fleet bases, and 1 pvp boot camp
territory control, Open pvp with limited number of players that can
enter the maps. no pvp queues.
with a unique crafting system, get the resources through pvp
to craft unique stuff.
this is how i imagine it to be. the unique pvp crafting system
would be the pvp end game type of content.
