Missions not giving items properly

System

A lot of the time any mission that gives an Item doesn't always give me the item, for example the stranded in space mission I didn't get the com code item. The pjem mission I didn't get the item and the academy mission I only intermittently get a data chip.

I have plenty of space in my bags and the hand in doesn't fail when I accept it I just don't get the item.
