Duty Officer: No information on mission status upon completion

Duty Officer Mission Title: Send a Representative to a Security Briefing for the Kassae Sector
Issue: I completed the Duty Officer mission "Send a Representative to a Security Briefing for the Kassae Sector."
I received the completed message while at Starfleet Academy, but when I went to the Duty Officer Interface, the Completed missions tab was grayed out.
Additionally, I am fairly sure I also lost my Duty Officer. He is not in sickbay, nor did he return to my DOFF Roster.
