Hmmm, that maint was a while ago. Seems like they forgot to turn her back on? EDIT: Got word that maint will work the magic to bring back the NPC. Now to see!

Sylen, I noticed that with the barb upgrade of Bestial Rage (more damage than sage/demon bestial rage - but it's a skill that doesn't damage on sage--not sure about demon). Sounds like there's a lot of text errors on this update. Fun times for future patches!

It's different per server per day. The Merchant (not Merchant Name, mind) in a couple cities (Sanctuary and Whetstone come to mind) has clues. He gives a free one every day for a random wandering NPC.
