Yes, a full inventory is also another possibility. Happened to me a few times... xD You have to make sure that you have a spot for the FB blood in your inventory before it says "failed" (before you leave the squad, or before the tabber leaves the squad).

For me, for some reason, that always looked like a person sitting in seiza. xD
