Seat of Torment Squad Leader DC = Instance Reset Bug

While in Seat of Torment last night for the weekly and BH quests, the squad leader disconnected from game due to Internet connectivity issue. All the mobs and towers were cleared to the bosses and squad is finalized and moving toward the BH boss Lord of Captivation.

At this point, when the leader dc'd, a timer appeared on my screen, but the squad panel still showed on my UI as if I were still in the squad. I timed out of the instance and ported to the Lothranis entry town area. I see the entire squad, minus the leader, also ported to the entry, thus resetting the instance.

Neither I nor anyone else could leave the "phantom" squad in order to reform a new squad. Each and every one of us were only able to leave the squad by logging off the character and logging back in, thereby losing all chi and buffs and apoth and guild base buffs and everything we had already prepared in order to complete the former instance.

Then we started reforming squad and as some players were unable to continue for various reasons, we had to find new players to fill the squad. This new squad included a new cleric, because our former squad leader was a cleric, so we were basically rendered unable to continue until we located the new cleric.

EDIT: I should add that the squad leader and I and others in the squad were located in the "Nightmare" area of the instance when this occurred.
I did not feel the treachery or inconstancy of a friend, nor the injuries of a secret or open enemy. I had no occasion of bribing, flattering, or pimping, to procure the favour of any great man, or of his minion; I wanted no fence against fraud or oppression: here was neither physician to destroy my body, nor lawyer to ruin my fortune; no informer to watch my words and actions, or forge accusations against me for hire: here were no gibers, censurers, backbiters, pickpockets, highwaymen, housebreakers, attorneys, bawds, buffoons, gamesters, politicians, wits, splenetics, tedious talkers, controvertists, ravishers, murderers, robbers, virtuosos; no leaders, or followers, of party and faction; no encouragers to vice, by seducement or examples; no dungeon, axes, gibbets, whipping-posts, or pillories; no cheating shopkeepers or mechanics; no pride, vanity, or affectation; no fops, bullies, drunkards, strolling prostitutes, or poxes; no ranting, lewd, expensive wives; no stupid, proud pedants; no importunate, overbearing, quarrelsome, noisy, roaring, empty, conceited, swearing companions; no scoundrels raised from the dust upon the merit of their vices, or nobility thrown into it on account of their virtues; no lords, fiddlers, judges, or dancing-masters.
