Lost something.......while refining?

Yeah weird I know. I was refining a lower level piece of gear to +2. Closed the window when I was done. Open my inventory window back up, and.......wow, no gear. o.o

It wasn't a serious lose, only something I was gonna use for an alt. But did anyone else have this issue? I know I didn't npc it cause......well, I wasn't npcing stuff......

But losing gear while refining it could cause a big problem if it is a bug......
