You can add sockets to crafted weapons and armor (as far as I know you can't add them to mob drop or NPC items.) Weapons can have a maximum of 2 sockets, armor 4 sockets.

Refining has nothing to do with the item's level. Refined weapons give an attack bonus, refined armor an HP bonus, refined ornaments a pdef, mdef or evasion bonus, depending on the type of ornament. You can see what the bonus would be for various refining levels by looking up the item on Perfect World Database.

As Mayfly said, you can add up to 2 total sockets to a weapon, and 4 total to armors.

HOWEVER, it's quite costly, and therefore not worth doing till late to endgame. that lvl. 5 weapon you have will be obsolete in days, or even hours, as you level past it; you can generally get from 1-20 in a few nights of playing, and you're going to be acquiring new weapons just as quickly. On top of that, the shards available to socket into low level weapons/armors don't give significant additional stats, making it even LESS cost effective. I would say not to bother with sockets until you're 80+, at least, and only truly epic gears then.

The same thing applies to refining; it costs 1-2 mirages per try, each worth roughly 10,000+ coins, with a low chance of success. And again, the low stat gain at lower levels make it pointless from a monetary point of view, as well as again the fast gear turnover rate.
Refining has nothing to do with the item's level.
The amount you gain from each+ is based partially on the grade of the weapon, which roughly corresponds to the level range it's in (ie. a lvl 5 weap is most likely a G1 weap). So it does sorta have something to do with the item's level >.> (I'm just picking nits here)
